This role offers a meaningful opportunity to make a genuine difference by enabling individuals to live in and take pride in their own homes, build independence, confidence and meaningful connections with dignity.
You will play a key role in enabling people to develop skills, engage with their communities and live empowered, fulfilling lives, exploring interests and achieving personal goals through both one-to-one and group-based coaching.
This is a permanent/full time/rota based/inc. every other weekend & sleep-ins.

What You'll Do
Coaching & Daily Living Support
- Coach and support individuals in daily living skills, including self-care, domestic tasks and maintaining a safe, healthy home environment.
- Promote independence through practical skill-building and encouraging personal choice and control.
- Support participation in community activities, employment opportunities, hobbies and social relationships.
- Encourage independence while respecting individual choice, dignity and rights.
Health & Wellbeing
- Promote healthy lifestyles, including nutrition, exercise and emotional wellbeing.
- Support with personal care and medication, where required.
Risk Management & Safeguarding
- Promote positive risk taking while ensuring the safety and wellbeing of individuals.
- Enable problem solving, complete risk assessments, incident reports and implement proactive support strategies.
- Maintain strong safeguarding practices, confidentiality and professional standards.
Communication & Record Keeping
- Maintain accurate, clear and timely records.
- Communicate effectively with colleagues, families and external professionals.
- Work collaboratively within a team to deliver high-quality, consistent support.
Professional Practice
- Uphold dignity, respect and individual rights, at all times.
- Demonstrate commitment to continuous learning and development.
- Maintain professional boundaries and contribute to team culture.
What We're Looking For
ESSENTIAL
- A compassionate, proactive, flexible, respectful and calm approach with commitment to empowering independence.
- Strong communication and recording-keeping skills.
- Ability to build positive relationships; engage individuals and groups.
- Experience/understanding of health & wellbeing.
- Awareness of safeguarding and good professional practice.
- Ability to manage workload, prioritise tasks and adapt to varied environments.
- Knowledge of community inclusion, equality and diversity.
- Commitment to professional standards and continuous learning.
DESIRABLE
- Experience providing personal care and supporting diverse needs.
- Experience in supporting adults with additional learning needs, learning disabilities or neurodiversity.
- Awareness of learning disabilities, Care Act and CQC requirements.
- Understanding of person-led and strengths-based approaches.
- Transferable skills.
- Current full UK driving licence and use of a vehicle (advantageous).
